Jubilee Home Care, where we care for all. We are a licensed and approved New Jersey Division of developmental Disability (DDD) Services Provider Agency. Services We Provide
Behavioural Support
Jubilee Home Care offers individual and group counseling, behavioral interventions, and diagnostic evaluations. Services include comprehensive assessments, the development of behavioral modification plans, caregiver training and supervision, and regular reassessments to ensure effectiveness.
Community Based Support
Jubilee Home Care provides personalized support for individuals, with or without a caregiver, both at home and in the community. Our goal is to boost independence, productivity, family dynamics, and community involvement through tailored, one-on-one assistance.
Individual Support
Individual Supports offer help to participants, with or without a caregiver, at home or in the community. This includes assistance with daily tasks, social skills, and self-care, promoting independence, productivity, and community involvement to improve quality of life.
Prevocational Training
Services offering learning and work experiences, including volunteer work, where individuals develop general skills for employability in integrated community settings. These services include training in communication, workplace conduct, following directions, task attention, problem-solving, and general workplace safety and mobility.
Career Planning
Jubilee Home Care offers personalized employment planning and support, helping individuals secure, retain, or progress in competitive jobs or self-employment. We aim to guide participants in choosing a career path and creating a plan to achieve integrated, competitive employment at or above the state’s minimum wage.
Interpreter Services, Speech Language and Hearing Therapy
The service helps participants improve communication within their community and access essential resources. It aims to enhance community engagement and resource utilization, ensuring participants can effectively connect and benefit from available support.
Respite
Services for participants who cannot care for themselves are offered short-term to relieve or replace their usual caregivers. These respite services can be provided in various durations, such as partial hours, hourly, or daily, and can be delivered in the participant’s home.
Community Inclusion
Services offered outside a participant’s home that support and assist in educational, enrichment, or recreational activities. These services aim to enhance community inclusion, providing opportunities for learning, growth, and engagement in various activities that foster a sense of belonging and participation.
Supported Employment
Services are designed to assist individuals in securing and sustaining employment in a competitive, customized, or self-employment setting within the general workforce. This includes personalized support to obtain a job and ongoing assistance to maintain it in an integrated work environment.
